A form to update the Director of either the submission or conclusion of a complaint, a whistleblowing report or a minor issue.
Outcomes

Outcome of a complaint
This form is given to the person who makes a complaint and the person the complaint is against at the end of the process to let them know the outcome.

This form is given to the person who raises a minor issue and the person the minor issue was raised about at the end of the process to let them know the outcome.
